Anyone - physicians, hospital discharge planners, patients, family members, friends or other community agencies - can make a referral to Visiting Nurse Association. One phone call is all that is necessary to start the process. We'll be glad to answer any questions you may have. Services from Visiting Nurse Association do require a physician's order.
If you meet certain income requirements you may be eligible for a government-sponsored program or for financial assistance from the Visiting Nurse Association. For more information please call 831-479-6633.
